What is a Brazilian Butt Lift?

The Brazilian butt lift (BBL) is a procedure designed to correct a flattened or sagging look by giving a round, taut, and perky appearance to the buttocks. The BBL procedure really has two components. The first component of a Brazilian butt lift is liposuction performed on the abdomen, back, bra-line, waist, and/or other “donor” areas. The goal of this is to obtain fat to place in the buttocks and to sculpt and shape the body to enhance the overall figure and proportions.

The second component of a Brazilian butt lift is the careful and meticulous placement of the collected fat cells into volume-deficient areas over the buttocks and hips in a superficial plane. A Brazilian butt lift can produce very subtle and natural results or very dramatic results, depending on patient preference.

What Makes a Good Candidate for Brazilian Butt Lift?

Good candidates for the Brazilian butt lift or BBL procedure are those with one or more of the following characteristics:

  • “Square” shaped body without much shape and definition to the waist, hips, and buttocks
  • A “violin” deformity where there are multiple ill-defined rolls over the body and a narrow-ill defined flattened buttock

What Are the Brazilian Butt Lift Surgery Details?

The Brazilian butt lift procedure at Nashville Plastic Surgery Institute takes two to three hours. Small access incisions are made for the selective removal of fat for future use for placement over the buttocks and/or hips. Incisions for Brazilian butt lift are generally very small, and in areas that are typically very concealed along the panty line and bra-line.

Once the fat is collected, this fat is prepared, then placed with a cannula into the buttock and hips, focusing on placement in superficial planes. A Brazilian butt lift involves adding fat over deficient areas as well as adjacent areas to ensure a smoothly contoured result.

What is Brazilian Butt Lift Recovery Like?

Brazilian butt lift recovery is generally one to two weeks. Patients are allowed to sit on the same day, but sparingly. At Nashville Plastic Surgery Institute, the Brazilian butt lift procedure makes use of a technique where the fat is placed into the hips and buttocks evenly and massaged into place, preventing the likelihood of distortion with sitting.

You will be placed in a special garment after Brazilian butt lift surgery. This garment is called a first stage garment and is worn during the initial healing stages. You will then be placed in a different garment (“Faja”) for the next phase of healing. With a Brazilian butt lift, generally, about 30 to 50 percent of the fat that is placed into the buttock is lost and does not “take.” This process is complete after about six weeks. It takes several months for all the swelling from the liposuction to resolve.

What Makes a Good Candidate for Buttock Augmentation with Implants?

Good candidates for a buttock augmentation include the following:

  • patients with a low BMI and little body fat, but with flattened buttocks, who desire fullness and roundness but do not have the donor fat due to low BMI
  • patients who have undergone massive weight loss with pronounced deflation and square shape
  • patients with a pronounced square-shaped buttock
  • patients who desire buttocks with very full projection but who lack the anatomy to obtain this with fat alone
  • patients who desire more dramatic proportions
  • patients who have had a prior Brazilian butt lift and desire a fuller shape, but do not have sufficient fat for another BBL (fat grafting) session.

What Are the Buttock Augmentation Details?

Buttock augmentation with an implant takes three to four hours. The procedure is performed with the patient under general anesthesia on an outpatient basis in a fully accredited surgical center.

First, liposuction is performed over the waist, abdomen, and/or bra-line and back. This is followed by buttock implant placement. The incisions are well concealed within the crease. Two incisions are made over each gluteal crease. After the pocket is made, the implant is placed with a Keller funnel for a no-touch technique, and the incision is closed in multiple layers with dissolving sutures. Fat grafting (minimal when compared to Brazilian butt lift) is then performed around the edges of the implant over the buttock and hips in order to enhance the result and to make the implant less visible.

What Implants Are Used for Buttock Augmentation?

The implants used for buttock augmentation are different from the implants used for breast augmentation. They have been in use for decades and are manufactured by a handful of companies. They are semi-solid silicone and are more durable than breast implants. They are soft feeling, natural, durable, and safe. Buttock implants come in varying shapes and sizes just as breast implants do. Implant selection will depend on your goals, wishes, and body type.

The three most common pockets for buttock implants are submuscular (under the muscle), intramuscular (within the muscle), and subfascial (under the fascia). When implants are placed under the muscle or within the muscle, this can lead to prolonged recovery and prolonged pain. Additionally, animation deformities can occur, and the implant does not fall to the lower portion of the buttock, which is often where volume is needed the most. Implants placed purely under the fascia and fatty tissue often have high rates of infection and implant loss.

The surgeons at NPSI utilize a unique technique called the “dual plane augmentation” (DPA), founded by Dr. Rolando Morales. With DPA, the lower third of the implant is within the muscle, forming a sling that stabilizes the implant, while the remainder of the pocket is under the durable muscle fascia. This approach decreases the implant visibility. This approach also lessens the likelihood of malposition and long-term pain.

We also utilizes fat grafting (similar to a Brazilian butt lift, but with less volume) with the buttock implant in order to enhance the shape and further decrease the visibility by allowing for a smooth transition between the implant and surrounding transition. This enhances the result by making it appear more natural.

What is Buttock Augmentation Recovery Like?

As with the Brazilian butt lift, you will go home on the same day and will be restricted from lying on your buttocks/back for two weeks. You are also restricted from sitting for two weeks. Recovery is generally two to three weeks. You will be seen by your surgeon initially fairly frequently following surgery, and then on a yearly basis to check your implants. You will be in a special garment after surgery for a total of three months. These garments can be worn under clothing.

What is Buttock Augmentation Recovery Like?

Some patients benefit from a composite buttock augmentation, which combines the effects of a Brazilian butt lift and implants. Your plastic surgeon will work with you to determine whether a Brazilian butt lift alone, implants alone, or composite is ideal.

You are restricted from sitting for two weeks. Recovery is generally two to three weeks.

Pain is reduced by the DPA (dual plane augmentation approach) for placement. Pain is generally treated well with pain medication in the initial period, followed by anti-inflammatory medicines.

There is always a risk of implant exposure, but with the DPA technique, this is very low. The highest risk of implant exposure is with the pure sub-fascial approach. Adhering to a strict no sitting and lying on your stomach for two weeks, as advised by your surgeon, reduces the risk of implant exposure.
